TERMS OF SERVICE

Membership Terms of Service

Payment Schedule

  • Members can choose their payment date for automatic billing.
  • Monthly payments will occur on the same date each month.

Payments

  • A valid credit or debit card is required to enroll as a member.
  • If a payment is declined, membership benefits will be suspended immediately.
  • If the initial 3-month commitment has not been completed, you will be charged for the remaining months.
  • To restore membership, an alternative payment method must be provided.

General Terms and Disclosures

  • Initial commitment of 3 months, followed by month-to-month membership with a 30-day written notice of cancellation.
  • After the initial 3 months, you can cancel your membership by providing a 30-day cancellation notice. You will still be responsible for the payment of the membership month leading up to the exit date.
  • Service credits roll over up to 30 days beyond the month charged, except in the Face/Body Membership.

No Show and Late Cancellations

  • The No Show/Late Cancellation policy applies to members.
  • Appointments must be canceled 24 hours in advance, or the appointment is considered a used treatment.

Transferability

  • Membership benefits are non-transferable and non-refundable.
  • Memberships are granted on an individual basis and are non-transferable to other members of the member’s household or friends.

Freezing Membership

  • Members can freeze their membership for one month by completing a Freeze Membership Request Form via the Client Portal. The frozen month will be added to the end of the membership term and billed accordingly.

Exclusions

Other services may not replace those included in the membership program but may be purchased at a separate discounted rate as mentioned in the membership terms. Discounts are not valid on the purchase of gift certificates unless otherwise stated.

Minors

  • Minors must have the Membership Agreement entered into by a parent or guardian.

Termination

Evolve Wellness Spa reserves the right to terminate or deny re-enrollment of a member at any time in its sole discretion.

Membership Fees

Membership fees may increase, but members will receive notice about any increases. Prices will not increase during the current contracted membership.
